Parent/Guardian Acknowledgment & Release As the parent or legal guardian of the above-named participant, I acknowledge and agree to the following: Voluntary Participation I give permission for my child to participate in the 239 Lady Cheetahs flag football program, including but not limited to practices, games, tournaments, team travel, and related activities. Assumption of Risk I understand that participation in flag football involves certain risks, including but not limited to collisions, falls, physical exertion, and unpredictable actions of other participants. I voluntarily assume all such risks on behalf of my child. Medical Authorization In the event of an injury or emergency, I authorize the coaching staff or designated representative to seek necessary medical treatment for my child. I understand that I am responsible for any costs incurred as a result of medical services provided. Release of Liability I hereby release, waive, and hold harmless the 239 Lady Cheetahs, its coaches, staff, volunteers, sponsors, affiliates, and facility partners from any and all liability, claims, or causes of action arising out of participation in this program. Insurance Responsibility I understand that the 239 Lady Cheetahs do not provide health or accident insurance and that it is my responsibility to carry adequate coverage for my child. Media Consent I grant permission for my child’s name, image, and likeness to be used in team photos, videos, social media, and promotional materials unless I opt out in writing. Signature & Consent I have read and fully understand this waiver and release. I certify that my child is in good health and physically capable of participating in team activities.
